Director Michael Day selected a small-town playhouse to film the hospital scenes, casting lead actor Jake Abel alongside supporting performers Greg Nutcher and Thomas Weissmuller. Weissmuller had begun acting at that same playhouse in the early 1970s. Fog of War was Weissmuller's first film, prompting a pause and a shared, "that's so cool" moment as the camera was being framed at the start of filming.
