In the opening of the film several people are playing basketball. Clarence (Ernie Hudson) attempts a shot. It’s clearly visible that the hoop has a net attached. However, when the camera angle shifts and the ball is seen passing through the rim, the net is absent.

In the lead-up to 3:23 the players move towards a basket fitted with a red, white and blue net. A shot is taken at that net, but when the ball strikes the rim (at 3:23) the net is missing.

In the vault sequence, Redvale says "There you go. A hundred million in hundred-dollar notes", yet the shot shows only two suitcases stuffed with cash, whereas in reality it would take one million notes to make up that sum and would occupy far more space — likely filling the vault.

Despite being Catholic, the priest recites the Protestant version of The Lord's Prayer, even saying "For thine is the kingdom ..." Catholics do not recite the final two lines.

Marcus says he's diabetic and signals low blood glucose by asking for juice or a chocolate bar. A nurse appears to check his blood glucose and then gives him an injection. There would have been no reason to administer a shot, particularly if it was insulin, since that would have lowered his blood glucose further. He should instead have been offered a source of quick glucose, such as the juice he requested. As he is alert and able to eat or drink, an injection is not required.

While Marcus is on the office's roof, he attempts to fire his pistol and finds it has run out of ammunition. With the magazine empty, the pistol's slide would normally lock back, indicating there are no rounds remaining. (If the final cartridge in the chamber had been a dud, he could have cycled the action by hand to load the next round.)

When Marcus kills Floria in the lift scene, he fires the guard's automatic, which is not fitted with a silencer. He then picks up Floria's silenced automatic lying by her head and seems to place his pistol at her waist. The subsequent shot shows a silenced automatic beside her head — the one he had just taken.
